Requirements

• Visa Form filled out and duly completed by typewriter or in legible print. Available at the headquarters of the consular offices or in the service portal
   

• One (1) photograph size 2 x 2 inches , front and white background.
   

• Original passport  with a minimum validity sufficient for the stay in the Dominican Republic.
   

• Verbal or Diplomatic Note   from the Diplomatic Mission, Consular or International Organization, requesting the visa and indicating:

1. Applicant's name

2. Nationality

3. Passport number

4. Diplomatic rank or position held

5. Purpose of the trip

Application Fee:    USD$150.00

Legalization:    USD$100.00  (By document)

Translation:    USD$40.00  (By document)
